Deliver skilled interventions to improve patient functional independence for geriatric clients in ALFs home settings. Implement selected components of care and interventions in a technically competent manner while communicating with families, physicians, and other health care team members. Maintain timely, thorough documentation of services in the medical record. Work as a PRN provider with the ability to make your own schedule and agree or disagree to patient assignment. Competitive compensation of $70 per transaction applies to this role within the outpatient therapy at home provider network.
