Job Summary
Clinical Case Manager providing therapeutic and case-management services to residents in a supportive housing setting. This role combines clinical therapy, life-skills development, and housing-support services to help individuals with serious mental illness and co-occurring conditions achieve housing stability and recovery goals. Responsibilities include conducting mental health assessments and screenings, developing individualized service plans, delivering individual therapy (as appropriate), facilitating psychotherapy and life-skills groups, applying evidence-based practices (e.g., Motivational Interviewing, trauma-informed care), and documenting services in required systems. Also coordinates access to healthcare, employment, education, housing resources, and community supports; advocates for residents and navigates complex systems; collaborates with an interdisciplinary team; supports admissions and eligibility processes; and maintains compliance with program standards. Requires a Master’s degree in a related behavioral health field and active Utah licensure (LCSW/LCMHC/LMFT), along with case-management experience and strong clinical skills. The role emphasizes recovery-oriented, trauma-informed, person-centered care within a Housing First framework, with a focus on building meaningful therapeutic relationships and helping residents achieve stability and independence.
Required Qualifications
- Master's Degree in Social Work, Clinical Mental Health Counseling, Marriage and Family Therapy, or a related behavioral health field
- Active Utah licensure as: Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Marriage and Family Therapist (LMFT)
- Case management experience
- Strong clinical assessment, documentation, and treatment planning skills
- Excellent interpersonal, communication, and problem-solving abilities
