Job Summary
Collaborate with residents to identify needs and develop individualized service plans while maintaining housing stability and complying with housing requirements. Assist clients with housing recertifications, documentation, and navigation of complex systems including healthcare, employment, and benefits eligibility. Facilitate life skills groups, coordinate transportation, and utilize assertive outreach strategies to encourage community participation and self-directed recovery goals. Maintain accurate documentation in KIPU, HMIS, and Azure systems while ensuring compliance with HIPAA and 42 CFR Part 2. Work within a multidisciplinary team to support individuals with serious mental illness and co-occurring conditions toward housing stability and long-term success.
Required Qualifications
- Case management experience
- Strong interpersonal, communication, and problem-solving skills
- Ability to work effectively with individuals experiencing serious mental illness and co-occurring disorders
- Strong organizational skills and attention to detail
- Ability to document services accurately and professionally
- Proficiency with Microsoft Office and electronic documentation systems
- Reliable attendance and punctuality
Desired Qualifications
- Bachelor's Degree in a human services-related field
- Active Utah Social Service Worker (SSW) license
- Experience working with individuals experiencing serious mental illness
- Experience with substance use disorders
- Experience working with individuals experiencing homelessness
- Familiarity with Housing First and recovery-oriented models of care
- Experience facilitating groups and community-based services
- Familiarity with KIPU and HMIS systems
