Job Summary
Conduct social assessments, interview clients to gather case histories, and prepare intake reports. Evaluate client strengths, skill needs, and eligibility for social welfare benefits while developing individual action plans. Facilitate referrals to legal, medical, financial, and housing resources, and provide crisis intervention and emergency shelter services. Supervise staff and volunteers, deliver workshops on life skills and substance abuse treatment, and monitor client progress and program effectiveness. Maintain contact with external agencies and track program statistics for evaluation.
Required Qualifications
