Job Summary
Conduct home visits to assess care needs, provide counseling to clients and relatives, and prepare applications for health insurance. Evaluate care plans, coordinate with physicians and other organizations, and handle administrative tasks such as performance recording. Serve as a trusted contact for families regarding daily care and broader family questions. Work within a specialized team offering flexible scheduling and remote options.
