Job Summary
Provide care and services to patients in private homes, Skilled Nursing Facilities, and assisted living facilities as a contributing member of the interdisciplinary team. Conduct psych-social assessments, interventions, and coordinate with nurses, social workers, chaplains, volunteers, and CNA leadership. Manage visits, documentation, and meetings while prioritizing urgent needs, utilizing independent communication skills via phone, email, and written channels. Requires current RN licensure, valid driver's license, and ability to learn electronic medical records systems. Earn $55-65/hr with a $2500 sign-on bonus, 401k match, and PTO.
Required Qualifications
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
- Valid driver's license
- Auto insurance
- Reliable transportation
- Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
- Ability to learn an electronic medical records system
