Job Summary
Lead and coordinate 245D-compliant care services for individuals with disabilities in St. Paul, MN. Develop and implement individualized service plans in line with 245D regulations, coordinate care resources, and conduct regular assessments to monitor client progress. Facilitate communication among clients, families, and service providers; maintain accurate documentation per state guidelines; provide staff training on 245D standards; advocate for clients’ rights within the community and service systems; stay updated on regulatory changes to ensure compliant, high-quality service delivery.
Required Qualifications
- Bachelor’s degree in social work, human services, or a related field preferred
- Minimum of 2 years of experience in coordinating services for individuals with disabilities
- Knowledge of 245D regulations and best practices in service delivery
- Strong organizational and communication skills, both verbal and written
- Current Minnesota driver’s license and reliable transportation
- Compassionate demeanor with a commitment to client advocacy and empowerment
- Certification in CPR and First Aid is a plus
