2026-2027 VOLUNTEER/MENTOR (Lunch Buddy, School Based Volunteers, and Mentors)

Complete the attached Mentor Form and Parent Mentoring Agreement Form if selected to serve as a bona fide mentor for students in the school setting. Submit the finalized documents to the Parent Liaison at the school your child attends. Allow at least 7 business days for processing. This application applies to all applicants desiring to volunteer in the school setting, including roles as a Lunch Buddy, Classroom volunteer, or Athletics mentor.
