Job Summary
Provide assistance to individuals and families through the case management model, including intakes, assessments, and developing plans of care. Develop and facilitate therapeutic support groups while creating health and wellness programming for the community. Offer one-on-one support to help clients remove barriers and choose a healthy lifestyle. This role requires a Bachelor of Social Work or related field with a minimum of three years in addictions or mental health services. Applicants must be current internal employees and Band members.
Required Qualifications
- Bachelor of Social Work, Psychology, Health Sciences, Community Services, or a related field
- Diploma in Social Services, Addictions and Mental Health, Community Services, Health Sciences, or a related discipline combined with relevant experience
- Minimum three (3) years of experience providing addictions, mental health, wellness, or community support services
Desired Qualifications
- Experience working within a First Nation organizations, health centre, social service agency, or community-based program
