Job Summary
Evaluate and treat hospice patients in a home setting in Lake City, FL, maintaining thorough daily documentation in the EMR. Communicate with families, physicians, and other healthcare professionals while bringing your own equipment, including smart devices and a blood pressure monitor. As a part-time, per diem therapist, you select your schedule and accept or decline patient assignments from an on-call pool, with rates up to $120 per evaluation. This role offers flexibility for experienced or new graduates holding an active Florida Physical Therapy license to deliver preventative geriatric therapy in residential home environments.
Required Qualifications
- Graduate of an accredited college or university with a Doctoral/Masters/Bachelors degree in Physical Therapy
- Actively Licensed as a FL Physical Therapist
- DPT or Physical Therapist (DPT,PT,RPT)
- Home setting experience
- Hospice patient services experience
- Weekend and Holiday Shifts availability
- PRN work availability
