Job Summary
Diabetes Nurse role within the Diabetes Wellness Program and Wellbeing, reporting to Clinical Lead Supervisor. Provide supportive, collaborative chronic disease management for diabetes in one-on-one or group settings. Key duties include intake/assessment, care planning using standard tools, EMR documentation, developing community education programs, supporting education for clients and families, coordinating with primary care, and ensuring adherence to College of Nurses of Ontario guidelines. Requires either a BScN or nursing diploma and CN Ontario registration, plus Certified Diabetes Educator credential. 35 hours/week, full-time; wage $70,400–$88,000 per year. Based in Ohsweken, Ontario, Canada; on-site work with no remote arrangement.
Required Qualifications
- Graduate of a Bachelor of Science in Nursing OR Graduate of a Diploma in Nursing
- Must be registered with the College of Nurses of Ontario, entitled to practice
- Certified Diabetes Educator
