Job Summary
Indigenous Transitions Facilitator provides discharge planning coordination from hospital to home, serving as the first point of contact for community members being discharged. The role partners with the Wellbeing Department to research, develop and educate on strategies to improve discharges, strengthens linkages to wrap-around services, coordinates referrals across the circle of care, and plans education or training to support clients’ living arrangements post-discharge, with seamless handover to the Home and Community Care Case Manager.
Required Qualifications
- Bachelor’s or Master’s degree in Nursing, Occupational Health or Social Work
- Minimum of 2 years’ experience in a hospital setting including case management and system navigation
- Knowledge of ECFAA, FIPPA, PHIPA, HCCA, FLTCA
- Must have First Aid, CPR and WHMIS
- Computer skills in Microsoft Office; experience with EMR systems such as PS Suites and Clinical Connect preferred
