$100/eval- Conyers, GA 30012- Physical Therapist(DPT,PT,RPT) : in a home setting-PRN

Evaluates and treats hospice patients in a home setting while communicating with families, physicians, and other healthcare professionals. Maintains daily, thorough documentation of services in the EMR. Requires owning your own equipment including smart devices and a blood pressure monitor. This PRN role offers flexible scheduling with assignments sent on an as-needed basis, allowing you to accept or decline cases based on availability. Compensation is up to $100 per evaluation.
