Job Summary
Conduct comprehensive in-person assessments in members' homes, hospitals, and community settings to evaluate medical, functional, behavioral health, and social needs. Develop person-centered service plans, collaborate with healthcare providers and community partners, educate members and caregivers on available resources, and advocate for vulnerable populations. Support individuals in maintaining their independence while ensuring high-quality, compliant care through the MassHealth Long-Term Services and Supports (LTSS) program. Perform assessments for Adult Day Health, Adult Foster Care, Group Adult Foster Care, and Personal Care Attendant programs. Work Monday-Friday, 8:00 AM-5:00 PM, with eligible business travel reimbursed at the current IRS federal mileage rate.
Required Qualifications
- Registered Nurse (RN)
- Massachusetts Long-Term Services and Supports (LTSS) program knowledge
- Clinical assessment skills
- Ability to conduct comprehensive in-person assessments
- Experience evaluating medical, functional, behavioral health, and social needs
- Ability to develop person-centered service plans
- Collaboration with healthcare providers and community partners
- Ability to educate members and caregivers on available resources
- Advocacy for vulnerable populations
- Ability to support individuals in maintaining independence
- Ensuring high-quality, compliant care
- Monday-Friday schedule availability (8:00 AM-5:00 PM)
- Ability to manage own daily schedule
- Eligible business travel
- Massachusetts residency or ability to serve the state of Massachusetts
