Job Summary
Conduct comprehensive in-person assessments for MassHealth Long-Term Services and Supports (LTSS) programs, including Adult Day Health, Adult Foster Care, Group Adult Foster Care, and Personal Care Attendant. Evaluate medical, functional, behavioral health, and social needs in members' homes, hospitals, and community settings to determine clinical eligibility. Develop person-centered service plans, collaborate with healthcare providers, educate members and caregivers on available resources, and advocate for vulnerable populations. Manage a designated geographic region with eligible business travel reimbursed at the current IRS federal mileage rate. Work Monday-Friday, 8:00 AM-5:00 PM, with a hybrid remote schedule.
